- Although some private insurers may reimburse the insured person directly for any payments to Everhart Primary Health Care, understand that Medicare and Medicaid will not accept claims submitted by a private individual for health care services received from a nonparticipating health care provider.
- Contact your health insurance provider to determine if payments made to Everhart Primary Health Care for health care services are reimbursable or if they may apply toward your deductible or out-of-pocket expenses.
- You may request a receipt for payments, diagnostic (ICD-9/10) and/or procedural (CPT) codes for any services you receive at Everhart Primary Health Care.
- It is your responsibility to obtain, complete, and submit any forms needed for such reimbursement or application to your deductible/out-of-pocket expenses.
